Please read all conditions carefully before confirming your agreement
- 1The period for use of equipment is 30 days only. Please return it on expiry of 30 days.
- 2The equipment shall at all times remain the property of Trust "Shree Matunga Gujarati Seva Sangh".
- 3The equipment shall remain at the sole risk of user family, and any loss or damage shall be borne by them.
- 4Equipment are only for the use of patients only.
- 5Ensure that no unauthorized transfer or diversion of the equipment occurs.
- 6Please do not remove, alter, disfigure or cover up any numbering or lettering displayed on the equipment.
- 7Return the equipment, along with Deposit Receipt, on or before the due date and inform the office.
- 8In case the equipment is returned on public holidays or after office hours, please inform the concerned authority.
- 9Please produce the Deposit Receipt for refund of equipment deposit which is essential.
